INTERNAL AFFAIRS COMPLAINT FORM
To formally submit this Internal Affairs Complaint, the completed form must be sent through the forums by private message.
Submission Instructions:
1. Copy the completed Internal Affairs Complaint Form in full.
2. Open the forums private messaging system.
3. Create a new private message addressed to:
IAD: Command
4. Use the following subject line format:
Internal Affairs Complaint - Your Name5. Paste the completed complaint form into the body of the private message.
6. Attach or link any supporting evidence, if available, including screenshots, video footage, audio recordings, documents, or witness statements.
7. Submit the private message to the IAD: Command group.
Important Notice: Complaints should be submitted as soon as reasonably possible after the incident. The complainant should provide accurate, complete, and truthful information to assist the Internal Affairs Division with reviewing the matter. Complaints that are incomplete may still be reviewed, but missing information may delay the investigation process.
Confidentiality Notice: Internal Affairs complaints are reviewed by authorized personnel only. Do not post complaint forms publicly on the forums. All complaints must be submitted privately to the IAD: Command group.
